The Ministry of Defence has officially commenced a full investigation into the Ghana Air Force Z-9 helicopter crash that claimed eight lives on Wednesday, August 6, 2025.
This was announced by the by Acting Defence Minister Dr. Cassiel Ato Forson in a statement issued on Monday, August 25.
According to him, a high-powered Investigation Board, chaired by the National Security Coordinator, has formally begun its work with technical support from the United States Air Force.
The Board draws membership from the Ghana Air Force and the Aircraft Accident and Incident Investigation and Prevention Bureau of the Ghana Civil Aviation Authority (GCAA).
The Ministry revealed that visiting investigators from the U.S. Air Force will provide advisory and technical assistance, while the Board may also consult additional experts—both local and international—to ensure a comprehensive inquiry.
A preliminary team, which was immediately constituted on the day of the crash, has already collected evidence and is expected to submit its findings to the Board today.
The tragic incident occurred in the Adansi Akrofuom District of the Ashanti Region, killing senior government officials and security officers, including Defence Minister Dr. Edward Omane Boamah, Environment Minister Dr. Ibrahim Murtala Muhammed, Acting Deputy National Security Coordinator Alhaji Limuna Muniru Mohammed, and NDC National Vice Chairman Dr. Samuel Sarpong.
The rest were Samuel Aboagye, Deputy NADMO Director-General; Squadron Leader Peter Bafemi Anala; Flying Officer Manin Twum-Ampadu; and Sergeant Ernest Addo Mensah.
The Defence Ministry emphasized that the Investigation Board has a strict 30-day mandate to establish the cause of the crash and make recommendations.
